摘要

“肝者，罢极之本”，心力衰竭（心衰）乏力产生的关键环节在于肝的生理特性失常，提出以“补肝之体、和五脏气、疏肝之郁”为心衰乏力的治疗总则。常用逍遥散健脾养血柔肝，六味地黄丸补益肝肾，或一贯煎固肾精、调和五脏；生黄芪、生山药、生甘草顺应其生发之性，白芍、酸枣仁养肝血，敛心阴，以养魂安神。心衰后期，常佐以佛手、柴胡、香橼、生麦芽等以疏肝木之郁。通过调肝以达到改善心衰乏力的目的。

Abstract

This study explores the key link of fatigue in heart failure （HF） from the perspective of the "liver as the root of tolerance to fatigue". It holds that the key lies in the abnormal physiological characteristics of the liver as the "root of tolerance to fatigue"， and proposes the general treatment principle for HF-related fatigue as "tonifying the liver's body， harmonizing the

of the five

zang

organs， and soothing liver

stagnation". Commonly used drugs include

Astragali Radix

，

Dioscoreae Rhizoma

， and

Glycyrrhizae Radix

et Rhizoma

， which correspond to their nature of promoting growth；

Paeoniae Radix Alba

and

Ziziphi Spinosae Semen

to nourish liver blood and consolidate heart

yin

， thereby nourishing the soul and calming the mind；

Xiaoyao Powder

to strengthen the spleen， nourish the blood， and soften the liver；

Liuwei Dihuang Pills

to tonify liver and kidney； or

Yiguanjian

to consolidate kidney essence and harmonize the five

zang

organs. In the late stage of HF， drugs such as

Citri Sarcodactylis Fructus

，

Bupleuri Radix

，

Citri Fructus

， and

Hordei Fructus Germinatus

are often added to soothe liver

stagnation. By regulating the fundamental physiological functions of the liver as the root of tolerance to fatigue， this approach aims to improve fatigue related to HF.
