摘要

腰椎间盘突出症是最常见的以腰腿痛为主诉的临床疾病。其推拿治疗多选择受累脊柱节段及膀胱经、胆经，理筋整复、疏通经脉。近年以股内收肌群、腰大肌为施治部位的足三阴经筋拨筋法被广泛应用于腰椎间盘突出症的治疗。足三阴经筋腰腹股部以腰大肌、股内收肌群为主体，足三阴经筋拨筋法符合中医“阳病治阴”“后病前治”“腰病治腹”的基本原则，足三阴经筋拨筋法可调整腰椎-骨盆-髋复合体力学平衡。筋骨理论、经脉经筋理论是中医推拿治疗筋伤病症的基本理论，临床宜从筋骨理论、经脉理论及经筋理论角度，构建“以筋骨为基础的经脉经筋体系”，以形成立体的调骨、理筋、通脉相结合的腰椎间盘突出症推拿诊疗框架。

Abstract

Lumbar intervertebral disc herniation （LIDH） is the most common clinical condition presenting primarily with low back and leg pain.

therapy is mainly based on the affected spinal segments and the bladder and gallbladder meridians， aiming to regulate tendons， correct misalignment， and unblock meridians. In recent years， sinew-plucking method in the foot three

yin

myofascial meridians， which targets the psoas major and hip adductor muscles in the lumbar， abdominal， and thigh regions， has been widely applied in the treatment of LIDH. This method conforms to the basic TCM principles of "treating

yin

for

yang

disorders"， "addressing the front for back disorders"， and "treating the abdomen for lumbar conditions"， and can help restore the mechanical balance of the lumbar-pelvic-hip complex. The theories of sinews and bones， meridians， and myofascial meridians constitute the foundational framework for TCM

therapy for sinew injuries. Clinically， it is recommended to construct a "meridian and myofascial meridian system based on the theory of sinews and bones" to form a three-dimensional

diagnostic and therapeutic framework for LIDH that integrates bone regulation， sinew management， and meridian unblocking.
