摘要

间质性肺疾病（ILD）是一组异质性强的呼吸系统疑难病，本病目前缺乏单一有效的治疗手段，部分药物可及性差。基于“可治疗特征（TTs）”的个体化管理策略，可为ILD患者提供“以人为本”的中西医管理框架，有利于实现多学科协作、院内院外协作、医患合作的疾病管理模式，使患者最大化受益。ILD-TTs肺部方面主要包括ILD病因评估，是否存在急性加重、感染、气道黏液高分泌、进展性纤维化、低氧血症以及咳嗽呼吸困难评估；肺外方面主要包括ILD相关疾病，如肺动脉高压、胃食管反流、心理状态、营养情况、心血管疾病、肺癌、糖尿病等；危险因素/行为方面主要包括吸烟、依从性、饮食情况、活动情况、家庭和社会支持情况。根据ILD的每类TTs，形成基于循证医学证据的干预方案及路径，可充分发挥中西医结合的优势。

Abstract

Interstitial lung disease （ILD） is a group of highly heterogeneous respiratory disorders， which currently lack a single effective treatment， and the accessibility of some drugs is poor. The individualized management strategy based on "treatable traits （TTs）" provides ILD patients with a "people-centered" framework for integrated traditional Chinese and Western medicine. This approach facilitates a disease management model that emphasizes multidisciplinary collaboration， cooperation between in-hospital and out-of-hospital teams， and doctor-patient cooperation， allowing patients to benefit to the greatest extent. The pulmonary aspects of ILD-TTs mainly include evaluation of ILD etiology， whether acute exacerbation， infection， airway mucus hypersecretion， progressive fibrosis， hypoxemia， and cough or dyspnea are present. Extrapulmonary aspects primarily involve ILD-related conditions， such as pulmonary hypertension， gastroesophageal reflux， psychological status， nutritional status， cardiovascular diseases， lung cancer， and diabetes. The risk factors/behavioral aspects mainly include smoking， adherence， diet， physical activity， and family and social support. For each type of TTs in ILD， intervention programs and recommendations based on evidence-based medicine are provided， making full use of the advantages of integrated traditional Chinese and Western medicine. The application of this scheme and model in ILD requires further research to optimize and validate its effectiveness.
