摘要

基于《黄帝内经》“土曰备化”理论探讨胃癌脾虚毒结核心病机的演变与证治。脾土失于备化是胃癌发生发展的基础，也是促进胃癌核心病机在不同病程阶段演变的关键。胃癌的病机演变为，早期脾气虚损、痰湿互结；中期脾阴不足、瘀热内盛；晚期脾阳不振、癌毒侵蚀。基于此，将“健脾解毒法”的论治思想贯穿于胃癌的全程治疗，早期健脾、祛痰毒，中期滋脾阴、化瘀毒，晚期温脾阳、抗癌毒。

Abstract

Based on the "earth responsible for thorough transformation" theory from

Huangdi's Internal Classic

Huangdi Neijing

）， this study explores the core pathogenesis evolution and treatment of gastric cancer， characterized by spleen deficiency and toxin accumulation. The loss of the spleen's transformative function is the fundamental cause of gastric cancer development and the key to the evolution of its core pathogenesis at different stages. The pathogenesis of gastric cancer evolves as follows： in the early stage， there is spleen

deficiency and the interaction of phlegm-dampness； in the middle stage， there is spleen

yin

deficiency with the internal accumulation of blood stasis and heat； in the late stage， there is spleen

yang

deficiency and the erosion of cancerous toxins. Based on this， the therapeutic approach of "strengthening the spleen and eliminating toxins" is applied throughout the treatment of gastric cancer. In the early stage， the treatment focuses on strengthening the spleen and eliminating phlegm and toxins； in the middle stage， nourishing spleen

yin

and resolving blood stasis and toxins； and in the late stage， warming spleen

yang

to fight against cancer and toxins.
