摘要

目的

观察振针助力针法联合加巴喷丁治疗气滞血瘀型带状疱疹后神经痛（PHN）的疗效及对血清炎症因子、5-羟色胺（5-HT）水平的影响。

方法

选取2023年6月—2024年6月昌邑市人民医院收治的PHN患者82例为研究对象，按照随机数字表法分为对照组、观察组。2组均给予加巴喷丁口服，对照组采用常规针刺法治疗，观察组采用振针助力针法治疗，每天针刺1次，均连续治疗2周。对比2组疗效，治疗前后疼痛视觉模拟评分（VAS）、匹兹堡睡眠质量指数（PSQI）评分、汉密尔顿焦虑量表（HAMA）评分、中医证候积分，以及炎症因子白细胞介素（IL）-6、降钙素基因相关肽（CGRP）、5-羟色胺（HT）水平，记录2组治疗期间发生的不良反应。

结果

观察组总有效率高于对照组（

0.05）。治疗后，2组VAS评分、HAMA评分、PSQI评分、中医证候积分、IL-6、CGRP、5-HT均较治疗前降低（

0.05），且观察组低于对照组（

0.05）。观察组不良反应发生率低于对照组（

0.05）。

结论

振针助力针法联合加巴喷丁治疗PHN气滞血瘀证疗效显著，可缓解患者疼痛和焦虑，改善睡眠，减轻炎症反应，降低5-HT水平，效果优于常规针刺法联合加巴喷丁治疗。

Abstract

Objective

To observe the therapeutic effect of the vibrating-needle-assisted acupuncture combined with gabapentin on patients with postherpetic neuralgia （PHN） of

stagnation and blood stasis type， and its impact on serum inflammatory factors and 5-hydroxytryptamine（5-HT）levels.

Methods

A total of 82 PHN patients were randomly assigned into a control group and an observation group using a random number table.The control group was treated with conventional acupuncture，while the observation group was treated with the vibrating-needle-assisted acupuncture once daily.Clinical efficacy，pain assessed by the visual analogue scale（VAS），Pittsburgh Sleep Quality Index （PSQI），Hamilton Anxiety Scale（HAMA），traditional Chinese medicine（TCM）syndrome scores，serum levels of interleukin-6（IL-6），calcitonin gene-related peptide（CGRP），and 5-HT were compared before and after treatment.

Results

The total clinical effective rate in the observation group was higher than that in the control group（

0.05）.After treatment，VAS，HAMA，PSQI，TCM syndrome scores，IL-6，CGRP，and 5-HT levels decreased in both groups compared with baseline（

0.05），and values in the observation group were lower than those in the control group（

0.05）.The incidence of adverse reactions in the observation group was lower than that in the control group（

0.05）.

Conclusion

The vibrating-needle-assisted acupuncture combined w

ith gabapentin is significantly effective for PHN of the

stagnation and blood stasis type.It can alleviate pain and anxiety，improve sleep，reduce inflammatory responses，and lower 5-HT levels，showing superior efficacy compared with conventional acupuncture combined with gabapentin.
