摘要

胡元会教授认为，持续性心房颤动病机以四维病理要素“虚、瘀、痰、火”的动态演变为核心，虚损为本，瘀、痰、火壅滞为标，形成了“因虚致实，实邪伤正”的恶性循环。胡教授临证主张分型论治，气阴两虚证治以益气养阴、宁心止悸，方用自拟芪珀生脉汤加减；心血瘀阻证治以活血通脉、息风止颤，方用自拟二虫通脉汤加减；痰瘀互结证治以化痰活血、通脉止悸，方用自拟礞石通脉汤加减；痰火内扰证治以化痰降火、镇惊安神，方用自拟天礞羚角汤加减，并善用青礞石坠痰降火。临床灵活化裁，标本兼顾以调和阴阳、复脉止悸。

Abstract

Professor HU Yuanhui holds that the pathogenesis of persistent atrial fibrillation is centered on the dynamic evolution of four pathological factors "deficiency， blood stasis， phlegm， and fire". Deficiency constitutes the root， while blood stasis， phlegm， and fire obstruction represent the manifestations， together forming a vicious cycle in which "deficiency leads to excess， and excess further damages healthy

Qipo Shengmai Decoction

with modifications. For heart blood stasis obstruction， the treatment focuses on activating blood circulation， unblocking the vessels， and extinguishing wind to relieve tremors， using the self-formulated

Erchong Tongmai Decoction

with modifications. For phlegm and blood stasis syndrome， the strategy is to resolve p

hlegm， promote blood circulation， unblock the vessels， and relieve palpitations， using the self-formulated

Mengshi Tongmai Decoction

with modifications. For phlegm-fire disturbing internally， the treatment focuses on resolving phlegm， draining fire， calming fright， and tranquilizing the mind， using the self-formulated

Tianmeng Lingjiao Decoction

with modifications， with good use of

Chloriti Lapis

to direct phlegm downward and clear fire. In clinical application， flexible modification is emphasized， addressing both root and manifestation to harmonize
