摘要

肉芽肿性乳腺炎是一种发生在非哺乳期或非妊娠期的乳腺慢性炎症疾病，其发病率逐年上升。“气脉常通”即人体阴阳调和、气血津液运行通畅的健康状态，是养生防病的根本原则。本文从“气脉”角度探讨本病防治，认为气脉失畅、滞塞不通为本病核心病机，其中脾肾两虚、气脉虚滞为本；气滞郁火、气脉闭阻为标；痰瘀胶结、正虚络损为变。根据GM临床表现及气脉不通的病机特点，治疗当以“通”为要，补脾益肾充气脉，调气通络清气脉，化痰祛瘀通气脉，并依据患者病情临证加减，使气脉常通、阴平阳秘。

Abstract

Granulomatous mastitis （GM） is a chronic inflammatory disease of the breast that occurs during non-lactation or non-pregnancy periods， and its incidence is increasing year by year. The concept of "

-pulse continuity" refers to a healthy state in which

yin

and

yang

are harmonized and

， blood， and body fluids circulate smoothly， representing a fundamental principle for health preservation and disease prevention. This paper explores the prevention and treatment of this disease from the perspective of the "

-pulse" theory. It is proposed that impaired

-pulse circulation and obstruction constitute the core pathogenesis： spleen and kidney deficiency with

-pulse insufficiency and stagnation form the root；

stagnation with constrained heat and obstruction of the

-pulse represent the manifestation； and phlegm-stasis congealment with deficiency of healthy

and collateral damage represents disease transformation. Based on the clinical manifestations of GM and the characteristics of

-pulse obstruction， treatment should prioritize "dredging". Therapeutic principles include tonifying the spleen and kidney to replenish the

-pulse， regulating

and dredging the collaterals to clear the

-pulse， and resolving phlegm and removing blood stasis to restore

-pulse circulation. Treatment is modified according to individual conditions， with the aim of maintaining

-pulse continuity and achieving

yin

yang

balance.
