摘要

直肠癌是常见的恶性肿瘤之一。腹泻是直肠癌进展中的常见症状，直肠癌未治疗阶段，其腹泻由癌毒侵犯直肠，肠道通降功能失司所致。直肠癌的各种治疗手段也是导致腹泻的重要因素，如放疗后腹泻与放射性直肠炎相关，其核心病机为湿热内蕴；术后腹泻与低位前切除综合征相关，其核心病机为脾肾不固，兼夹痰湿、血瘀、气滞；化疗后可出现化疗相关性腹泻，其核心病机为阴阳失和、寒热错杂。治疗直肠癌相关腹泻旨在动态把握疾病病机随治疗变化的规律，分阶段辨证论治，治以抗癌解毒、清热利湿、补益脾肾、调和寒热、行气、活血、化痰等法，通过灵活调整选方用药，调畅气机，恢复肠道的正常功能。

Abstract

Rectal cancer is one of the common malignant tumors. Diarrhea is a common symptom during the progression of rectal cancer. In the untreated stage， diarrhea is caused by c

ancer toxins invading the rectum and dysfunction of the intestine's descending and transmitting function. Various treatment modalities for rectal cancer are also important factors leading to diarrhea. For example， post-radiotherapy diarrhea is associated with radiation proctitis， with the core pathogenesis being the internal accumulation of dampness and heat. Postoperative diarrhea is related to low anterior resection syndrome， with the core pathogenesis characterized by insecurity of the spleen and kidney， accompanied by phlegm-dampness， blood stasis， and

stagnation. Chemotherapy-related diarrhea may occur after chemotherapy， with the core pathogenesis involving disharmony of

yin

and

yang

and a mixture of cold and heat. The treatment of rectal cancer-related diarrhea aims to dynamically grasp the changes in pathogenesis during the course of treatment and to implement stage-based syndrome differentiation and treatment. Therapeutic methods include anticancer detoxification， clearing heat and promoting dampness elimination， tonifying the spleen and kidney， harmonizing cold and heat， promoting

circulation， activating blood， and resolving phlegm. Through flexible adjustment of prescriptions and medications， this approach seeks to regulate

movement and restore normal intestinal function.
