目的

观察加味桃红四物汤中药溻渍联合压力治疗下肢静脉性溃疡（VLU）气虚血瘀证的临床疗效。
方法
2
选取2024年7月—2025年2月于中国中医科学院望京医院就诊的VLU患者72例，按照随机数字表法分为观察组和对照组，各36 例。对照组予压力治疗、创面常规换药、健康教育；观察组在对照组的基础上予加味桃红四物汤中药溻渍治疗。疗程均为4周，或至溃疡愈合（完全上皮化）。分别于治疗前及治疗2、4周，对比2组生活质量评分［慢性静脉功能不全生活质量问卷（CIVIQ）评分、静脉疾病临床严重程度评分（VCSS）］；于治疗前及治疗4周，对比2组血清生长因子［血小板衍生生长因子（PDGF）、表皮生长因子（EGF）和转化生长因子-β（TGF-β）］水平，血清细胞外基质因子［基质金属蛋白酶抑制因子1（TIMP-1）、基质金属蛋白酶2（MMP-2）］水平。观察记录2组治疗期间发生的不良反应，比较临床总有效率。
结果

观察组总有效率为91.7%，高于对照组的80.6%（

=0.021）。治疗2、4周后，2组CIVIQ评分均较治疗前升高（

0.05），VCSS较治疗前降低（

0.05），且观察组CIVIQ评分、VCSS均优于对照组（

0.05）。治疗4周后，2组血清PDGF、EGF、TGF-β水平均较治疗前升高（

0.05）。治疗4周后，2组血清TIMP-1、MMP-2水平均较治疗前降低（

0.05）。2组患者均未发生严重不良反应。
结论

加味桃红四物汤中药溻渍治疗VLU疗效显著，可提高患者生活质量，改善血清生长因子、细胞外基质因子水平，且安全性高。
Abstract
Objective

To observe the clinical efficacy of herbal compress therapy with modified
Taohong Siwu Decoction
combined with compression therapy in the treatment of venous leg ulcers （VLU） with
qi
deficiency-blood stasis syndrome.
Methods

A total of 72 patients with VLU who visited Wangjing Hospital of the China Academy of Chinese Medical Sciences between July 2024 and February 2025 were enrolled and randomly divided into an observation group and a control group （
n
=36 each） using a random number table method. The control group received compression therapy， routine wound dressing changes， and health education. Based on the treatment of the control group， the observation group additionally received herbal compress therapy with modified
Taohong Siwu Decoction
. The treatment duration was 4 weeks or until ulcer healing （complete epithelialization）. The Chronic Venous Insufficiency Questionnaire （CIVIQ） score and Venous Clinical Severity Score （VCSS） were assessed before treatment and at weeks 2 and 4. Serum levels of growth factors ［platelet-derived growth factor （PDGF）， epidermal growth factor （EGF）， and transforming growth factor-β （TGF-β）］ and extracellular matrix factors ［tissue inhibitor of metallop
roteinases-1 （TIMP-1） and matrix metalloproteinase-2 （MMP-2）］ were measured before treatment and at week 4. Adverse events during treatment and the total effective rate after treatment were also recorded.
Results

The total effective rate in the observation group was 91.7%， which was higher than 80.6% in the control group （
P
=0.021）. At weeks 2 and 4， CIVIQ scores in both groups were significantly increased compared with those before treatment （
P
<
0.05）， while VCSS scores were significantly decreased （
P
<
0.05）； both CIVIQ and VCSS scores in the observation group were superior to those in the control group （
P
<
0.05）. After 4 weeks， serum levels of PDGF， EGF， and TGF-β in both groups were significantly increased compared with baseline （
P
<
0.05）， and were higher in the observation group than in the control group （
P
<
0.05）. Meanwhile， serum levels of TIMP-1 and MMP-2 in both groups were significantly decreased compared with baseline （
P
<
0.05）， and were lower in the observation group than in the control group （
P
<
0.05）. No serious adverse events occurred in either group.
Conclusion

Herbal compress therapy with modified
Taohong Siwu Decoction
demonstrates significant efficacy in the treatment of VLU， improving quality of life， regulating serum growth factors and extracellular matrix components， and exhibiting a high safety profile.
