胡荫奇教授认为“湿浊”“热毒”“痰瘀”是痛风发病的关键病理因素,并根据病情发展,将其分为痛风急性发作期、痛风间歇期、慢性痛风性关节炎期、痛风性肾病期四个阶段。胡荫奇教授提出“病证结合,以病统证,分期制宜”的学术主张,临证时认为,痛风急性发作期宜清热利湿、解毒活血,方选自拟清利活血解毒汤加减;痛风间歇期宜燥湿运脾、化痰祛浊,方选自拟痛风2号方加减;慢性痛风性关节炎期宜祛湿泄浊、化痰散瘀,方选四妙散合桃红四物汤加减;痛风性肾病期宜健脾益肾、化湿通络,方选参芪地黄汤合四妙散加减。同时擅长使用经验药对,如秦皮与土茯苓、山慈菇与徐长卿、山药与山茱萸,两药相合协同增效。
Professor HU Yinqi considers "damp turbidity", "heat toxin", and "phlegm stasis" to be the key pathological factors in the development of gout. According to disease progression, gout is divided into four stages: the acute attack stage, the intermittent stage, chronic gouty arthritis, and gouty nephropathy. Professor HU proposes the academic principle of "integrating disease and syndrome, taking the disease as the primary framework, and implementing stage-based treatment". In clinical practice, during the acute attack stage, treatment should focus on clearing heat and promoting diuresis, removing toxin, and activating blood circulation, using a modified
Qingli Huoxue Jiedu Decoction
. During the intermittent stage, therapy should aim to dry dampness and strengthen the spleen, resolve phlegm, and eliminate turbidity, using a modified Gout Formula No. 2. In the stage of chronic gouty arthritis, treatment should focus on eliminating dampness and draining turbidity, resolving phlegm, and dispersing blood stasis, using a modified combination of
Simiao Powder
and
Taohong Siwu Decoction
. In the stage of gouty nephropathy, treatment should aim to strengthen the spleen and tonify the kidney, resolve dampness, and dredge the collaterals, using a modified combination of
Shenqi Dihuang Decoction
and
Simiao Powder
. Professor HU also emphasizes the use of empirical herb pairs, such as
Fraxini Cortex-Smilacis Glabrae Rhizoma, Cremastrae Pseudobulbus Pleiones Pseudobulbus-Cynanchi Paniculati Radix et Rhizoma

Dioscoreae Rhizoma-Corni Fructus
, which act synergistically to enhance therapeutic efficacy.
